Report with the following documents in original and a photocopy on the above reporting date

1. This offer letter
2. Completed Admission Form^#
3. Declaration Form (if necessary)^#
4. Medical Examination Report^#
5. SC/ST/OBC-NCL/PD certificates (**). Non-creamy layer OBC certificate valid for current financial year 2015 – 2016.^#
6. Completed and signed Application Form which you printed out while applying(original + copy)
7. Class 10 certificate as proof of Date of Birth (**) (original + copy)
8. Proof of photo ID if not covered in item 7 [Any ONE of the following: Driving License, Aadhar Card, College/School ID card or Passport]
9. Proof of admission fee payment to the bank
10. XII standard mark list and certificate (**) (original +copy)
11. Transfer Certificate from the School in Original
12. JEE (Advanced) Admit Card/ DST-INSPIRE letter (**) (original + copy)
13. Address proof for opening a bank A/C
14. Six recent Passport size photographs
15. Duly filled and signed Anti Ragging Undertaking Form(https://www.iisertvm.ac.in//pages/anti_ragging_initiatives)
